Social Prescriber

  • Conducts comprehensive assessments to understand patients’ non-medical needs and concerns
  • Identifies social, emotional, and practical issues impacting health and well-being
  • Develops personalised support plans and sets goals with each patient
  • Refers patients to counselling services, support groups, and mental health charities.
  • Support social isolation and loneliness interventions
  • Assists patients with housing issues, including navigating housing services and accessing support.
  • Offers guidance on employment opportunities, job training, and educational programs.
  • Encourages healthy lifestyles through diet, exercise, and smoking cessation advice and refers patients to wellness programmes.
  • Support for carers – connecting them with respite care, support groups and carer-specific resources
  • Facilitates local events in the community for the Patients of Salutem PCN
  • Follow up and ongoing support.
